The American Joint Committee on Cancer (AJCC) has
released both the AJCC Cancer Staging Form Supplement as well as the AJCC Histology and Topography Code Supplement to the AJCC Cancer Staging Manual, Eighth Edition. These resources are available on www.cancerstaging.org.
The AJCC Cancer Staging Form Supplement includes 104 printable staging forms for each distinct
staging system published by the American College of Surgeons (ACS).
These forms may be used by physicians to record
data on T, N, and M categories; prognostic stage groups; additional prognostic
factors; histologic grade; and other important information. These forms may be
useful for recording information in the medical record and for communicating
information from physicians to the cancer registrar.
The staging forms may be used by individuals
without permission from the ACS or the publisher. This document may be printed
or made into PDF forms.
The staging forms cannot be sold, distributed, published, or
incorporated into any software (including any electronic record systems),
product, or publication without a written license agreement with ACS. With the
exception of adding form fields to the PDF, the forms cannot be modified,
changed, or updated without the express written permission of ACS.
The
AJCC
Histology and Topography Code Supplement includes the
complete list of histology codes and topography codes for each of the 108
distinct staging systems published in the AJCC
Cancer Staging Manual, Eighth Edition.
This
resource for data collectors presents a complete list of topography and
histology codes eligible for collection of TNM, prognostic factors, registry
data collection variables and/or AJCC Prognostic Stage Groups.
